2026 Camp forms will be updated once the lottery is underway. These forms can only be submitted once an online registration has been completed and paid in full.
- Interested CIT’s must submit the same forms as campers, after completing the CIT application.
- The Medication Log is only required for campers/CITs that have a prescription for medication that they will be bringing to camp.
- In accordance with Maine DHHS camp licensing, a copy of each campers’/CITs’ current immunization records must be submitted along with the required camp forms to attend camp.
Records can be emailed to the Camp Director or mailed to the address below, fax is not available.
